Website privacy

This section covers this website only. Information about people we support is governed by our Notice of Privacy Practices, below.

What this site collects

When you submit the referral form we collect the details you type into it: your name, role, organization, email address, phone number, preferred contact method, county, the services you are interested in, your funding source, your timeframe, and any notes you add.

Our web server also records ordinary technical information with each request — IP address, browser type, the page requested and the time. This is used to operate the site securely and to rate-limit the referral form against abuse.

What this site does not do

• No advertising or tracking cookies
• No third-party analytics profiling you across other sites
• No selling, renting or trading of your information, ever
• No social media tracking pixels

The one embedded third-party element is the Google Map on our contact page, which loads only when you scroll to it and is subject to Google's own privacy policy.

How referral submissions are handled

Referral form submissions are delivered by email to our intake team through a transactional email provider. We keep them only as long as needed to respond to your inquiry and to maintain our own records of referrals received.

Why we do not collect health information online

Our referral form deliberately asks about you, the person making the referral — not about the person who needs support. It does not ask for a client's name, date of birth, diagnosis, Medicaid number or any clinical detail.

This is a deliberate design decision. Ordinary web forms and email are not secure channels for protected health information. By keeping that information off the website entirely, we remove the risk rather than manage it. We collect what we need securely by phone once we are in touch.

Please do not include health information in the form's notes field or in an email to us. If you have already done so, call us on (301) 996-0992 and we will handle it appropriately.

HIPAA Notice of Privacy Practices

As a healthcare provider, Loyal Healthcare maintains a Notice of Privacy Practices describing how medical information about the people we support may be used and disclosed, and how they can get access to that information. It covers rights including the right to inspect and copy records, to request amendments, to an accounting of disclosures, to request restrictions, and to file a complaint.

Non-discrimination notice

Loyal Healthcare complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, or sex. Loyal Healthcare does not exclude people or treat them differently because of race, color, national origin, age, disability, or sex.

Loyal Healthcare provides

• Free aids and services to people with disabilities to communicate effectively with us, such as qualified sign language interpreters and written information in other formats (large print, audio, accessible electronic formats).
• Free language services to people whose primary language is not English, such as qualified interpreters and information written in other languages.

Filing a grievance

If you believe that Loyal Healthcare has failed to provide these services or discriminated in another way on the basis of race, color, national origin, age, disability, or sex, you can file a grievance with our Civil Rights Coordinator.
