Four services designed to work together, delivered across Laurel and Prince George's County, Maryland. Most of the people we support use more than one.
Personal Supports
Flexible one-to-one support in your own home and community, scheduled around your life, for people who do not need or want a residential placement.
Nursing Support Services
Registered Nurse assessment, clinical care planning and medication delegation — the clinical backbone that lets people with complex health needs live in the community safely.
Transportation Services
Door-to-door transport with trained, background-checked drivers — because a support plan that cannot get someone to their appointment is not a support plan.

Residential plus nursing
A community home is only as safe as the clinical oversight behind it. Pairing Community Living with Nursing Support Services is what makes placement possible for people with real medical complexity.
Personal supports plus transportation
One-to-one support that cannot reach a day program, a job or a doctor is support on paper only. Transportation is what turns a plan into a week.
Supports before placement
Personal Supports at home often prevents a residential placement from becoming necessary before anyone is ready for one. It is the service most families should ask about first.
Paying for services
Funding, in plain terms
Most of our services are authorized through Maryland Developmental Disabilities Administration waivers — most commonly the Community Pathways Waiver, the Family Supports Waiver and the Community Supports Waiver. Which one applies depends on the person's eligibility and budget, and your Coordinator of Community Services can confirm it.
Not DDA-eligible yet? We accept private pay for Personal Supports and Transportation, which lets families put something in place while an application is pending. Waiting without support is often what turns a manageable situation into a crisis.
